Background & aim: Secondary abdominal pregnancy is extremely rare obstetrical complication with high maternal and perinatal mortality. Secondary abdominal pregnancy is defined as a condition where the embryo or fetus continues to grow in the abdominal cavity after its expulsion from the fallopian tube. Case Report:ThisReport: This case is a 22 years old G3P1L1A1 at 39 weeks of period of gestation, presented with decreased fetal movement, pain in abdomen and bleeding per vagina since one day. On abdominal examination uterus was 36 weeks, relaxed, transverse lie and fetal heart was absent. Ultrasonography showed oligohydrominosOligohydramnios, placenta Placenta previa ,Previa,transverse Transverse lie Lie and fetal Fetal demiseDemise. While operating for cesarean, secondary abdominal pregnancy was identified. Placenta encasing the fetus in sac was seen attached to the fundus and right lateral aspect of uterus by a thick vascular stalk and also adhesions to omentum and bowel. This mass was separated by cutting and ligating the connecting vascular stalk. The placental mass along with dead fetus was removed and hemostasis achieved. Conclusion: Advanced abdominal pregnancy is an extremely rare life-threatening condition and requires a high index of suspicion. It is associated with high perinatal mortality which may be attributed to placental disruption and abnormal placentation.
